OP (Organization Performance) Survey for Six Sigma
The OP (Organizational Performance) for Six Sigma Survey was created by Dr. Janice M. Johnson to measure the effectiveness of Six Sigma programs. This validated survey can be used alone, or it can be used in conjunction with other validated survey and questionnaire instruments.
Licensing Terms and Conditions
An educational or commercial license is required to use the OP (Organizational Performance) for Six Sigma Survey. The following applies in both license cases.
- Aside from the following explicit items, no part of OP (Organizational Performance) Survey for Six Sigma may not be modified or paraphrased.
- “Six Sigma” may be substituted with “Lean Six Sigma”
- “your Operating Segment” may be exchanged with “your functional unit”, “your department”, “your division”, or “the organization”.
- The intended purpose of the OP (Organizational Performance) Survey for Six Sigma (measuring the performance of organizations that have implemented Six Sigma or Lean Six Sigma programs) may not be changed or modified.
- Aside from administering the OP (Organizational Performance) Survey for Six Sigma for measurement, no more that one question within the survey may be published. This restriction includes but is not limited to dissertations, other scholarly works, and commercial use.
- The one specific question to be published will be stipulated in your license agreement.
- Educational and commercial licenses of the OP (Organizational Performance) Survey for Six Sigma only permit the sharing of descriptive and inferential statistics.
- The following must be clearly indicated on each administered page of the OP (Organizational Performance) Survey for Six Sigma: “Copyright (c) 2009 Janice M. Johnson, Ph.D. of the Johnson advantEDge Institute (www.johnsonadvantedge.com), All Rights Reserved”.
- All charts, graphs, and tables resulting from works that utilize the OP (Organizational Performance) Survey for Six Sigma must include the following in-text citation: (Johnson, 2009).
- All published works that utilize the OP (Organizational Performance) Survey for Six Sigma must include the following reference: Johnson, J. M. (2009). - Copies of all published works that utilized the OP (Organizational Performance) Survey for Six Sigma must be emailed to the Johnson advantEDge institute within 30 days of publication. Copies must be submitted in PDF format and emailed to the link provided in your license.

This license entitles the licensee to administer the OP Organizational Performance Survey for Six Sigma to 200 participants within 1 year.
This license entitles the licensee to administer the OP Organizational Performance Survey for Six Sigma to assess 50 employees who are employed by one business and who are located at the same business location (site) within 1 year. Please purchase additional licenses for each additional set of 50 employees, for each additional location (site), and for each additional business.
